Name

Here, you need to enter your legal name without titles like Mr, Mrs, Shri, Dr, etc. You also need to provide proof of your legal name. For that, you can upload a scanned copy of your passport, PAN card, driving licence, etc. You can check different documents that can be submitted as proof on the UIDAI website. You are only allowed to make minor changes to your name.

Gender

You have three options – Male, female and other

Age and Date of Birth

You need to enter your birth date in DD/MM/YYYY format. If your family members are unaware of their exact date of birth, you can enter an estimated age in years. If you have documents for verifying your birth date, tick on ‘verified. If you do not have documents for verification of your birth date, tick on ‘declared’. 

Address

If you visit the Aadhaar Enrolment Centre, carry original address proof for address verification. Be very vigilant while entering your address because that is where you will receive your Aadhaar card. If you wish to include your parents, guardian, or spouse’s name in your address tab, you can choose C/o (care of), D/o (daughter of), S/o (son of), W/o (wife of), or H/o (husband of). In this section, you can change or enter your e-mail ID and mobile number.

Relationship

If the Aadhaar card application is below 5 years of age, the name and Aadhaar number of the father, mother or guardian becomes compulsory.

Documents

You need to mention which all documents you will be providing while submitting your form. These documents could be Proof of Identity, Proof of Address, Date of Birth and Proof of relation.

Using introducer or HUF

If your identity or address verification is based on Head of the Family (HoF) or introducer, you should provide your Aadhaar or EID number under HoF or introducer tab.
