All about Voter ID Form 6

Citizens can now apply for a voter ID card by accessing the National Voter Service Portal (NVSP). Read on to know all about Voter ID Form 6.

The Indian voter ID card is an identification document issued by the Election Commission of India to adults residing in India reaching the age of 18 (and above), mainly used as proof of identity for Indian citizens when voting in a city, or state and national elections. Apart from election purposes, it could also be required to be shown as a basic identity proof for various purposes such as creating a bank account, affidavit of your Aadhar card and so on.

 

What is the NVSP Form 6?

The National Voter Service Portal (NVSP), a portal developed by ECI, now provides an online means for candidates to apply for a voter ID card easily. You can complete and submit NVSP Form 6 to claim your voter card.

National Voter Service Portal was launched by the Election Commission of India on January 25th, 2015 under the leadership of Chairman Dr. Abdul Kalam. NVSP Form 6 has been launched to provide a one-stop service for voters in India. 

These are the services provided by the NVSP:

  • Register for a voter ID card online
  • Find your name on the voter list
  • Check your constituency, congressional district and polling place
  • Request a change or correction to the voter ID card
  • Electoral officer and agent contact details at booth
  • Download your Aadhaar card and link it to your voter ID card.

How to apply for the voter ID card by submitting Form 6?

  • Go to the official National Voters Service Portal: https://www.nvsp.in/
  • Click ‘Forms’
  • Scroll down the page and click ‘Form 6’
  • It will redirect you to a new page
  • Provide information, including state, county and congressional district
  • Click ‘As
  • Scroll down and provide the remaining details
  • Upload the relevant documents
  • Submit the captcha code
  • Now, choose “Submit”
  • A receipt number will be generated
  • You can use this number to track your request

 

Guidelines for filling up the application NVSP Form 6

Who may submit an NVSP Form 6?

  • An Indian citizen who was 18 years of age or older on January 1 of the year in question for the revision of the electoral record. For instance, in the revision process using January 1, 2014 as the qualifying date, a person who turns 18 on January 1, 2014 (i.e., A person born on or before January 1, 1995) is entitled to enrol as an elector and can submit NVSP Form 6.
  • A registered voter who changes their primary address from the area in which they are currently residing.

When is the Form-6 submittable?

You can submit the application at any time of the year. It may be filed during the electoral roll revision after the constituency’s electoral roll has been officially published. The application must be turned in prior to the cutoff date for claims and objections. The above-mentioned time range receives the necessary exposure when the modification programme is introduced. The application must be submitted in a single copy during the revision programme. During the time period that we refer to as the non-revision period, applications must be submitted in duplicate.

Where to file NVSP Form-6?

  • The designated locations where the drafted electoral roll is posted (often polling stations), as well as the offices of the constituency’s Electoral Registration Officer and Assistant Electoral Registration Officer, are where the application may be made during the revision period. The application may also be submitted online on the Chief Electoral Officer of the relevant State’s website.
  • When the revision programme is not going on, the application can be filed only with the Electoral Registration Officer or can be filed online.

 

How do I complete the NVSP Form 6?

1.Request 

Requests should be sent to the electoral registration officer of the constituency in which you wish to register. The name of the constituency must be mentioned in the free space provided for this purpose.

2.Name

Names must appear on the list of voters and must provide a Voter Photo Identification Card (EPIC). Full name must be written in the first box, and last name must be written in the second box. If you don’t have the first name, just write the last name. Caste should not be mentioned except when the caste name is used as part of the voter’s first or last name. Honorary names like Shri, Smt. Kumari, Khan, Begum, Pandit, etc., should not be mentioned. Please mention your name in English and the official state language, if possible.

3. Age and date of birth

  • The applicant’s age at the start of the year in which the revised voter list will be used must be at least 18. Enter your birthdate in numeric form using the format dd/ mm/ yyyy in the space provided. The candidate’s birthdate documents must be submitted if they are between the ages of 18 and 21. As proof of your birthdate, you may affix a duplicate from one of the following documents:
  • Birth certificate issued by the city or county birth registry
  • Birth certificate of the last school (Government / Accredited) attended by the applicant or from any other accredited educational institution
  • If a person in Grade 10 or above is admitted, they must submit a copy of their Grade 10 transcript, if it has a date of birth as proof of date of birth
  • Marker board type 8 if date of birth is indicated
  • Marker board type 5 if date of birth is indicated
  • Indian Passport
  • PAN card
  • Driving license
  • Aadhar Letter issued by UIDAI.
  • In the absence of any of the above, a statement in the prescribed format given in Appendix – I (attached to the instructions) is made by one of the applicant’s parents (or by the professor in the case of voters of the transgender category) may be given. In case of presenting a parental affidavit as proof of age, the applicant will have to appear for verification in front of the booth level officer/voter registration officer/voter registration officer. Alternatively, if none of the above documents is available and both parents are not alive, the applicant can attach a certificate of their age issued by the relevant Gram Panchayat organisation or a member of the municipal committee related town/city level.
  • In the event that an applicant is over 21 years of age and appears to be a cabin-level officer / assistant voter registration officer/voter registrar, the declaration of such person’s age shall be considered as proof of age and no documentary evidence. Age must be indicated in the year and month completed on the date of eligibility, i.e. 1 January of the reference year in which the list of electors was revised.

4. Gender

Write your full gender in the space provided, for example. ‘Women’. Applicants can indicate their gender as “Other” when they do not want to be described as “Male” or “Female”.

5. State of residence

Please mention the place name like village/town, district, state.

6. Care/Of

In case the applicant is not married, the name of the parent must be mentioned. If the candidate is married, the name of the spouse must be stated. The options in the column that do not apply should be crossed out.

7. Place of permanent residence

  • Fill in the complete and complete postal address, including PIN, where you normally reside and wish to register, in the space provided. A copy of one of the following information must be attached as proof of normal residence:
    • current bank/ kisan/ postal book; or
    • meal card; or
    • Passport; or
    • Driver’s licence; or
    • Income Tax Order; or
    • Final Lease Agreement; or
    • Final Water/Telephone/Electricity/Gas Connection Bill for this address, in the name of the applicant, or the name of his next of kin such as parents, etc. ; or
    • Any post/mail/courier sent through India Post on behalf of the applicant at his usual residential address.
  • In the event a homeless person does not have any of the above documents, the issuer at the counter will travel at night to the address listed on nvsp form 6 to ensure that the homeless person is indeed homeless and sleeping at the location is listed as his address on Form 6. If cabin staff can verify that the homeless person actually slept there, documentary proof of whereabouts will not be needed. It usually takes more than a night for such a check.
  • In the event that a student resides in a place of study, such as a dormitory managed by an educational institution or elsewhere, that student will have the possibility of being registered as a voter at the place of departure. their origin with the number of parents or at the address of the boarding house/place of residence or temporary residence to continue their studies.
  • Courses taken by the aforementioned students must be recognised by the government/central board/state/universities/reputable universities, and these courses must last at least 1 year. This student who wishes to apply to the dormitory/mess must attach a certificate of good faith (form in Appendix II) from the Principal / Principal / Principal / Registrar / Dean of the institution of education attached to Form 6.

8. Details of family members registered in the voter list. 

Please give names and other details of immediate family members i.e. Father/Mother/Brother /Sister/Spouses residing at the same address. The names of any relatives other than those above should not be mentioned.

9. Declaration

  • Please indicate the date of your residence at the specified address. If you do remember the exact date, please mention the month and year in which you reside in this place.
  • If your name is already registered on the electoral roll of another constituency, please clearly write the previous address complete with the PIN. If you already have an EPIC (Voter Photo Identification Card), please include the card number (printed on the front) and the issue date (printed on the back) of the card in the space for that purpose. You can check your EPIC number on your state’s Chief of Elections website.

10. Other terms

  • Please fill in all the details requested in the application. Incomplete applications may be rejected.
  • Please also include your full name and address in the confirmation and notice.
  • Please provide your mobile phone number and email address in the form, this option is optional, as voter registrars may use your cell phone number and email address whenever needed.
